Intensity-modulated radiotherapy for head and neck surgeons.

چکیده
The development of intensity-modulated radiotherapy (IMRT) has played a major role in improving outcomes and decreasing morbidity in patients with head and neck cancer. This review addresses this vital modality with a focus on the important role of the head and neck surgeon. The technique as well as its benefits and points of caution are outlined, the definitions of tumor and treatment volumes are discussed, and the dose and fractionation are detailed. Following this are several sections dedicated to the role of the head and neck surgeon in the planning of both definitive and postoperative radiotherapy to the primary site and neck. There is a focus throughout on anatomic and surgical considerations; commonly encountered situations are illustrated. With a deeper understanding of this technique and their own pivotal contribution to target delineation, head and neck surgeons will be poised to expand their role and improve cancer care for their patients. © 2015 Wiley Periodicals, Inc. Dose Distribution Analysis of Rapid Arc and Intensity Modulated Radiotherapy Plan in Head and Neck Cancer
Introduction: Globally, intensity-modulated radiation therapy (IMRT) is considered as highly precise and accurate method of radiotherapy planning. This technique amplifies spatial dose distribution conformity by modulating the intensity of radiation beams in each sub-volume. Additionally, it can reduces the dose to surrounding critical organs and deliver the planned dose to targets with the nom...

In the Netherlands, about 2,300 people per year are diagnosed with a malignant tumor in the head and neck region. About 450 of them are referred to the Radboud University Nijmegen Medical Centre. Most of them undergo radiotherapy, either as a sole treatment, or in combination with surgery or chemotherapy. As part of the tailored radiation therapy most patients will be treated with Intensity Mod...
متن کاملClinical outcomes among patients with head and neck cancer treated by intensity-modulated radiotherapy with and without adaptive replanning.
BACKGROUND The purpose of this study was to determine the effect of adaptive replanning on clinical outcome among patients treated by intensity-modulated radiotherapy (IMRT) for head and neck cancer. METHODS Three hundred seventeen patients underwent IMRT with daily image-guidance for newly diagnosed squamous cell carcinoma of the head and neck to a median dose of 66 Gy (range, 60-74 Gy). Of ...
متن کاملIntensity-modulated radiation therapy for head and neck cancer.
Radiotherapy planning studies have confirmed dosimetric advantages of intensity-modulated radiation therapy over conventional and conformal radiation therapy. Utilization of intensity-modulated radiation therapy is ideal in head and neck cancer patients. Critical structures can be spared due to sharp dose gradients and limited organ motion with correct immobilization. Initial clinical results h...
متن کاملFLUENCE MAP OPTIMIZATION IN INTENSITY MODULATED RADIATION THERAPY FOR FUZZY TARGET DOSE
Although many methods exist for intensity modulated radiotherapy (IMRT) fluence map optimization for crisp data, based on clinical practice, some of the involved parameters are fuzzy. In this paper, the best fluence maps for an IMRT procedure were identifed as a solution of an optimization problem with a quadratic objective function, where the prescribed target dose vector was fuzzy. First, a d...
